  • All visitors must wear a face mask while in the building. A face shield is also required while in the building if you have NOT received all required doses of the Covid vaccine and have reached 14 days past required dosage. In order to be considered vaccinated, you must be able to submit proof of your vaccination status.  If you are unable to provide proof, you will be considered a non-vaccinated visitor.   Masks and shields are available for purchase at Thrifty White Pharmacy at Bethany on 42nd (please use the pharmacy door on the west side of the building), the administrative desk at Bethany on 42nd and at the administrative desk at Bethany on University, or you can purchase one from a supplier of your choice.
  • One to two visitors per resident/patient at a time. All visitors must follow facility Covid-19 guidelines on masking, face shield use and infection control.
  • Children must be supervised at all times by an adult. One child per visitation.  Children over the age of two and vaccinated children over the age of 12 must wear a mask. Non-vaccinated children over the age of 12 must wear a mask and face shield.
  • To keep residents healthy and safe please consider getting vaccinated as soon as you are able.  If you have not been vaccinated, please consider getting tested before you visit and bring proof of current COVID-19 testing results with you.  The 25th St. testing site offers rapid tests, so you can have the results in 15 minutes.  We prefer proof of a negative COVID-19 test that was taken within the past 5 days.  If you do not get rapid tested, we recommend that you get a PCR test on the same day every week so you get an idea of when you will be receiving the test results from the Department of Health in either ND or MN.  Free testing is available at the following locations:
    • 3051 25th Street S., Suite K, Fargo, ND, between Grand Junction Subs and Midwest Vision. This site is open from 8:00 am to 5:00 pm Monday through Friday, and no appointment is necessary. Former Thomas Edison Elementary School, 1110 14th Street South, Moorhead. This is saliva testing and the site is open Monday/Tuesday/Friday from 12:00 pm to 7:00 pm and Saturday/Sunday from 10:00 am to 4:00 pm.  Schedule Appointment with Vault Health.
  • Compassionate Care visits are allowed for residents and patients in isolation after visitor has received donning and doffing instructions and education. Visitor understands that if the compassionate visitation is for a resident/patient in isolation and/or non-vaccinated, the visitor should consult their provider prior to entering as the visitor may be at an increased risk of exposure.  We strongly prefer video visits or regular phone calls until a resident or patient isolation is done.
  • All visitors must sanitize upon entering the building, during their visit, and when exiting the building. Please bring personal hand sanitizer with you if you can.
  • All visitors must limit their visitation to only the room of the resident/patient they are visiting.
  • If the family member/person being visited is in a Semi-Private room, please visit with them in a common area outside of their room on their household/unit.
  • If the visitor is unvaccinated and/or the resident being visited is unvaccinated, the visitor will refrain from shaking hands, hugging, or close contact with residents during the visit and limit touching objects/items in their room.
  • Visitation will not be allowed if the visitor has had any of the following symptoms in the past 3 days: cough, sore throat, shortness of breath, muscle aches, headache, fatigue, chills, loss of taste/smell, nausea, vomiting, or diarrhea.
  • We will not allow visitation if you have been in close contact* with someone who is suspected of having or does have COVID-19, unless you have been cleared from isolation by the Department of Health.
  • We will not allow visitation if you have a temperature greater than or equal to 100 degrees Fahrenheit.

Additional Guidelines for Non-Vaccinated Visitors for Skilled Care:

  • Visitors who have NOT received the required doses of a COVID-19 vaccine can visit during the hours of 9:30am – 11:00am, and 2:45pm – 4:15pm, Monday – Saturday.
  • Non-vaccinated visitors will be required to visit by appointment during the times outlined above. Length of visitation will be in 30-minute time slots.
  • Non-vaccinated visitors will visit at a location specified by the facility. At Bethany on University, non-vaccinated visitation appointments will take place in the Terrace Dining Room.  At Bethany on 42nd,  non-vaccinated visitation appointments will take place in the Brad Heinz Celebration Hall.
  • Non-vaccinated visitor appointments are arranged by calling the social worker assigned to the resident/patient. Non-vaccinated visitors who wish to schedule a visitation appointment are asked to call by 3pm the day prior to the desired appointment.  Appointments are on a first-come, first-served basis.  Please leave a detailed voicemail message.

  • Non-vaccinated visitors will be required to wear a mask AND face shield. Non-vaccinated visitors must supply their own face shield and have prior to visit.  We have face shields available at the reception desk for $2.00, if you do not bring your own.
  • Non-vaccinated visitors will be required to refrain from shaking hands, hugging, or close contact with residents during their visit. Non-vaccinated visitors must keep masks and face shield on at all times.  They will be required to refrain from eating/drinking during the visit.
  • Non-Vaccinated visitors will be asked to refrain from unnecessary movement in and around the building while here for visitation.
  • Upon completion of the scheduled visit, staff will transport the resident/patient back to their room. Please request assistance by having the resident press their call button or please call the staff of the resident/patient.

Please be patient and allow extra time for staff to transport residents to and from visitation appointments as they are also caring for residents on the units/households.

*Close contact is defined as being within 6 feet or within the same  room for more than 15 minutes while not wearing recommended personal protective equipment (i.e. respirator, eye protection) or having direct contact with infectious secretions of a COVID-19 case (being coughed/sneezed on).
